No physician–patient relationship

Reading this site, submitting the callback form, or emailing our office does not create a physician–patient relationship. That relationship begins only when you are seen and accepted as a patient at one of our offices.

Appointment requests

A request submitted through this site is a request only — it is not a confirmed appointment. We will contact you during office hours to schedule. Because we cannot guarantee that a web submission is received or read promptly, anything time-sensitive should be handled by phone.
