Not a channel for medical information

This website is a general information resource. It is not a secure or HIPAA-compliant channel. Please do not submit diagnoses, test results, medication lists, or other detailed health information through the website form or by email. To discuss your care, call us at 424-301-7090.

Protected health information you share with us as a patient — in the office, in your chart, or through our patient portal — is governed by HIPAA and by our Notice of Privacy Practices, which is separate from this policy and provided to you at your first visit.

What this website collects

Information you give us. If you use the callback request form, we collect your name, phone number, and the short description of your reason for visit that you choose to provide. We use it only to contact you about scheduling.

Information collected automatically. With your consent, we use Google Analytics to understand how visitors use the site. This records information such as pages viewed, approximate location (city level), browser and device type, and how you arrived at the site. It is used in aggregate and we do not use it to identify individuals.

Our web host also keeps standard server logs, including IP addresses, for security and troubleshooting.

Cookies and your choices

The site sets no analytics or advertising cookies unless you accept them in the banner shown on your first visit. Declining leaves only the strictly necessary storage the site needs to remember that choice. You can change your decision at any time using the link at the bottom of any page, or by clearing this site's data in your browser.

Who we share information with

We do not sell your information, and we do not share it for advertising. Information is shared only with service providers that make the site work — our web host, Google Analytics (analytics), and the map provider that renders our location map — and only as needed for those functions. We may disclose information if required by law.

Your rights in California

Under the California Consumer Privacy Act and the Confidentiality of Medical Information Act, California residents may request access to the personal information we hold about them, request its deletion, and correct inaccurate information. We will not deny you care or charge you differently for exercising these rights.

Children

This website is not directed at children under 13, and we do not knowingly collect information from them through it.
